HOW TO MAKE AN APPOINTMENT
New Patients It is important that you notify your insurance company that you have a new physician before you come for your appointment. Their phone number should be on the back of your insurance card. We should also have access to your previous medical records from your former physician. We will be glad to help you obtain these records. Established Patients Please call the office at (415) 456-8180 to schedule your appointments as needed. PATIENT TAKE IN
How to get a Referral If you have an HMO insurance please get a referral from your primary care doctor. No referrals needed for PPO or Medicare insurances. You can always call us to check if we accept your insurance. How to get a Prescription Refill Please call during business hours for your prescriptions refills. We need to see you at least once a year to refill your prescriptions. Your Insurance We accept all insurances except Kaiser and worker's comp. However some Blue Shield covered California insurances don't cover us. When in doubt, always call ahead with your insurance information or check with the front desk when you come in. Payment and Billing Copayments and balances that are due must be taken care of at the time of the visit. We charge the insurance company first then collect the balance from patients. For cash-only patients, please call to inquire about your visit charge. We do prior authorizations for every procedure and help patients in every way we can to accommodate their needs. Patients can reach us via phone, fax or email for requests or questions. We are open during lunch time and answer calls between 8.30am-5pm non stop. If you call after hours, you can leave a voice mail for us. We will return your call the next business day. |
